Mississippi State University Faculty Evaluation The Office of Institutional Research has implemented the new Faculty Evaluation system for Fall 2006. Faculty Evaluation forms are available in Adobe .pdf format. Please print the desired form for your particular class description. Also, a sample copy of the evaluation output is available for your viewing. (Please note: the final output for your class will be using the scale of "5 to 1", "5" being Strongly Agree and "1" being Strongly Disagree, not "1 to 5" as on the sample sheet.)
