Brian, two years ago I had a student with a deformed right hand.  He could use one finger of it to strike enter and the space bar.  He used his left hand for all other keys.  I used a copy of an old textbook called Type with One Hand by Nina K. Richardson published by South-Western.  Copyright, 1959.  He was able to get 30 CWAM by the end of the year just like everyone else.  I made a copy of the lessons for him to practice at home.  He used the same computers that we did.  It was wonderful to see him do it!
