Aimee, as you know, the draft evaluation form was only a draft. The
intent of the first effort was to encourage brainstorming for
the initial evaluation form for the guidebook from anyone.

There are numerous questions that could be included but we should not to
impose on the reader for anything that is not relevant. The draft form
that was submitted was only a mere prototype.  BUT also remember,
depending on the group who is reading the guidebook at a particular
time(school, district, business, both national or international -
whoever), we should direct questions with some generality because of the
the vast world-wide audience.

Caution should be taken in designing the evaluation form so no one is
offended by terminology used in the publication whether in the
guidebook or the evalution form.  Plus the time required to complete a
brief effective form is very important.  In this manner, we will not
create an imposition for the reader and later we will receive valued
feedback. 

Aimee, we should be sure the guidebook places the web site address for the
guidebook in a very visible location.  There could also be some type of
cover letter to accompany guidebook copies that would introduce
Dr. Anderson and stating why the class played an important role in the
revision process.  I am aware of the current acknowledgement page.

Examples of other questions could be:

1.  Did you easily access a guidebook copy.

2.  Are you developing an initial technology plan?

3.  Please indicate the number of technology plans that were developed
for your entity prior to the current plan you are developing.

(AIMEE, for this questions numbers would need to be added, e.g. 0-1, 1-2,
3-4 or whatever is decided)

4.  How often would you recommend a revision of the guidebook?

5.  Taking into consideration the overall information provided in the
guidebook, how would you rate the publication?

6.  Were the topics provided in a sequential order to follow the process
of developing the intructional technology plan?

7.  Was the information provided appropriate for your task?

8.  Was the publication's written information presented in an professional
manner with clear communications?

9.  Will you refer to the guidebook often while developing or updating
your instructional technology plan?

10.  Would the guidebook, following your reading, be placed in a strategic
location in your library for future reference?

11.  Would you like to hear from us when the evaluation form process
is complete?

12.  What recommendation(s) would you suggest to improve the publication.

13.  Were your expectations, of the information provided by the guidebook,
met?

14.  Was there any type of assistance required, to understand the
suggested 
process as stated in the guidebook.

15.  Please use the following space for further inquiries.  You will
receive an inquiry response following review of your evaluation page.

Aimee, some of these may be redundant.  Sorry I do not have a
guidebook copy handy at the moment or my evluation form draft.
Hopefully, these will be helpful or to encourage more thinking.
