tkt8763-01: Assignment #9

For assignment #9 I created an online form that could be used in
conjunction with my school district's computer training program for the
community.  The program is called PACT (Parents Accessing Current
Technology).  The computer training sessions are held in the high school
lab one evening during the week.  These sessions start with basic computer
functions and the skills continue to build throughout each session.  

The form I created would be helpful in letting the session leaders know
who attended each session, how the sessions benefited them, what is needed
for future sessions, while building a database of personal information for  
community support purposes.

The form did not transfer to the Web very well--I tried several things but
was unable to make it look exactly like it did in Word 2000.  Hope this
information can be of help to you online course members.
