RecSports Waiver

RecSports requires members to sign a waiver, releasing the University from liability in the event of injury. This is a byproduct of us being a RecSport Club. You must sign this waiver to "be a member" and participate in club events. I can not do anything about this. It is a University requirement. If you have any concerns about the contents of the waiver, please let me know and I will pass them along to the appropriate people. You can print the waiver here: (DOC) or (PDF).

Sign this form and get it back to me ASAP by one of the methods below:

1) Hand it to me at a club ride.
2) Use campus mail to send it to me free:
